Is our documentation defensible and reflective of patient need?
Are we following up on audit results, implementing performance improvement plans or reacting to problems without a strong solution to avoid re-occurence?
If your QAPI plan feels like a checkboxâor isnât actively guiding improvementsâitâs time to recalibrate.
