  • Taking immediate action when patient health or safety is at risk.

 Surveyors expect your QAPI program to reflect the complexity of your agency and be applied across all services, including those provided by contractors. More importantly, they expect to see evidence that it’s actually working

  • Audit care delivery and documentation. Does the medical record reflect the quality of care provided?

  • Evaluate quality outcome measures. Where are you trending up or down?

  • Compare your performance. How do you measure up against the competition?.  

  • Collect honest feedback: What are patients, families and referrals sources saying?

  • Track with purpose. Do you have internal analytics to trend issues and highlight wins?

These steps will give you a baseline to understand where your agency stands, and where you need to go. They help you pinpoint your  high risk, high-volume and problem- prone areas.
